Certification for Continuing Education Courses
2017 Mid-Year Congress

To obtain Continuing Education certificates for courses that you attended at the 2017 Mid-Year Congress:

- Choose your course from the list below. You must complete a separate evaluation for each CE you attended.
- On the next screen, enter your email (the same email used on your meeting registration).
In order to take an evaluation, we must have your attendance slip on file for that course. Attendance slips were distributed at the beginning of each course, and should have been returned to the volunteer proctor at the end of each course. If the system does not validate your attendance, please contact INS@utah.edu for assistance.
- Answer all questions, then click on Submit Evaluation. All evaluations are anonymous (your name will not be associated with your responses).
- On the next page, click the Download Certificate button. Your certificate will be saved in PDF format. It may be saved to your desktop or your downloads folder, depending on your web browser's default settings.
